Residents outside of Laurel, Batangas (Subject to the availability of limited slots)

  1. Documents. Submit to Office of the Registrar the following:
    1. SHS Form 138 and 137 (at least with 3rd grading period)
    2. Barangay Clearance (optional for non-Laurel residents)
    3. Parent's Voter's I.D. (optional for non-Laurel residents)
    4. Birth Certificate (PSA/NSO/LCR)
    5. 2 pcs of 2x2 ID Pictuers (with name/white background) (original copies)
  2. Procedure:
    1. Fill out the Admission/Examination Permit Form (Form 1)
    2. Submit Form 1 to the Guidance Office.
    3. Get an examination schedule at OSA
    4. On the testing date. please present the test permit
    5. Wait for the posting of exam resutls posted in Municipality of Laurel website or official facebook page.
    6. Passers must secure and fill-out the Cumulative Record Form (CRF) from Guidance Office.
    7. Get the schedule of college interview from Guidance Office.
    8. After the college interview, the accepted applicants must return the CRF to the Guidance Office and get the enrollment requirements and schedule
3. Admission Procedures for Returnee Students
  1. Secure Clearance Form from Office of the Registrar
  2. Report to your college for advising and encoding of subjects.
  3. Photocopy of PSA/NSO/LCR Marriage Contract (if recently married)

ENROLLMENT REQUIREMENTS AND PROCEDURES

  1. FRESHMAN
    1. Documents. Submit the following documents to the Office of the Registrar:
      1. One (1) original and two (2) photocopies of Good Moral Certificate from the school last attended
      2. Two (2) Photocopy of PSA/NSO/LCR Birth Certificate (Note: bring the original for verification purposes)
      3. Original Form 137 or 138
      4. One (1) Photocopy of Drug Test Result
      5. One (1) original Barangay Clearance
      6. One (1) Photocopy of Parent's Voter's ID (for Laurel residents)
      7. One (1) Photocopy of PSA/NSO/LCR Marriage Contract (if married)
      8. Two (2) pieces 2x2 Pictures with white background
      9. Three (3) pieces 1x1 Pictures with white background
      10. One (1) long brown envelope and one (1) long white folder
      11. Resutls of Medical Examination
    2. Procedure
      1. Get the student number, encoded subjects, and have your photo taken from Office of the Registrar.
      2. Take the Student Registration Form (Form 2-SRF) from Office of the Registrar
      3. Present your Form 2-SRF to the instructor on the first day of classes.
    3. For Old Students:
      1. Proceed to your respective colleges/departments
      2. Get the pre-enrollment slip from the enrollment adviser of the College and proceed to the Dean's Office for encoding of subjects.
      3. Present the pre-enrollment slip to the Registrar's Office and get the Student Registration Form (Form 2-SRF)
      4. Present your Form 2-SRF to the instructor on the first day of classes.
    4. Enrollment Policy
      1. Enrollment is always on a semistral basis. The enrollment schedule follows the dates in the Academic Calendar issued by the Registrar's Office
      2. Only duly registered/officially enrolled students with Form 2-SRF will be included in the Master List of Students and will be allowed to attend classes. The Registrar's Office will furnish the instructors/professors with a copy of the Master List and will submit the same to the Commission of Higher Education (CHED).
      3. Studnets are given the (10) working days, after teh encoding of subjects to settle their accounts, to be included in the Master List which will be submitted to the Commission on Higher Education (CHED).
      4. Students must have proof of their machine validated Form 2-SRF.
